DENTAL SAVINGS CERTIFICATE
This certificate is redeemable for a Dental Savings Program, providing your customers and their immediate family with dental discounts ranging from 10% to 50% off the usual fees on specified services at over 24,000 participating dental providers nationwide. As a member, they are free to select any network dentist you wish. They simply make an appointment at their convenience by informing the dentist (or his staff) of their membership. They then pay the low program rates for the services performed. Their membership materials will include a schedule of services and rates and a listing of network dentists in their area. There is no limit to the amount of money they can save. THIS IS NOT INSURANCE.
To redeem the certificate, the customer simply mails or faxes it in with a $40.00 processing, postage and handling fee to receive their Dental Savings Program. They can also redeem their certificate online. Simple redemption instructions are clearly printed on each certificate.
